Are you a carer?

Do you look after a family member or friend who is unwell, disabled or frail?

If so please complete this form. Once you are added to our list of carers we will know about your busy life as a carer, which can affect your health. We can also try and be flexible with appointments as we will know about your commitments.

We offer appointments with the carers resource on a Thursday afternoon twice a month, if this is something you think would benefit from please contact the carers resource office on Tel: 01423 500555 or pop in to the surgery to fill in a referral card.

In addition to our medical support, there is a wealth of information on NHS Choices about carers and caring and the social and financial support available to you.

Although sending information across the Internet means that privacy of data cannot be absolutely guaranteed, our online forms offer high levels of security. The forms use HTTPS and TLS (using a 2048 bit certificate) to encrypt data sent to and from your computer when using the site and our server environment is PCI DSS certified - the highest standard of security associated with e-commerce and online finance solutions.

Form submissions are stored on the server for 28 days and are encrypted and secured with a private key known only by the GP Practice.

Should you have any concerns about sending your personal details using the web, please use one of the alternative methods offered by our organisation.
